In 1996, Health Essentials Co-Founders Kim Phan and Chris Mlot, MD formed GeriNet Medical Associates, a network of physicians who provide care for patients within skilled nursing facilities and to homebound residents. Recognizing the need for better post-acute care for patients and their families, three hospice organizations were born: Hospice Touch (2002), Gerinet Healthcare (2006), and Health Essentials Hospice (2012).
In 2007, Comfort Rx was established to provide pharmaceutical services to our patients. The following year, Health Essentials Durable Medical Equipment opened its doors to supply patients with the medical equipment needed to transform their home into an appropriate caregiving environment.
Today, Health Essentials is a full-service healthcare organization that specializes in providing post-acute care to the frail elderly population across California, Arizona, and Nevada. Its family of companies includes a medical group (GeriNet Medical Associates), Medicare-certified hospice services (Gerinet Healthcare, Hospice Touch, Inc., and Health Essentials Hospice), a pharmacy (Comfort Rx), and a durable medical equipment (DME) supplier (Health Essentials DME). Together, our family of companies provides patients and their families with compassionate care that is well coordinated, responsive, and of the highest quality.
In October 2012, the Health Essentials family of companies joined together upon receiving a growth capital investment from SV Life Sciences (SVLS) and Bessemer Venture Partners (BVP). Moving forward, Health Essentials is expanding its services into new markets and enhancing the coordination of care for the most medically complex patients. As a result, Health Essentials is uniquely positioned to engage with patients and their families, coordinate their care, drive quality outcomes, and reduce medical costs across the continuum of care.
