Coordinated Discharge – From the moment a patient is discharged from the hospital, there is a plan in place to support their transition to the skilled nursing facility, assisted living facility, or wherever they choose to receive hospice care
Same Day Admissions – The moment a patient is eligible for hospice, they can begin receiving care from us the same day. Our Nurse Practitioners meet with a patient's caregiver to complete the paperwork required to begin immediate care
24/7/365 Pharmacy – We have an onsite pharmacy available 24 hours a day, seven days a week, 365 days a year. We also provide overnight medication delivery to wherever our patient receives care, and our pharmacists are directly involved with after-hours triage
Durable Medical Equipment (DME) – Our goal is to make the patient as comfortable and pain-free as possible. By providing state of the art medical equipment to a patient's home, we specialize in transforming their place of comfort into a place of care. Regardless of the length of care, each patient receives a greater quality of life through improved functionality and independence
Nurse Practitioners (NP's) on call – Our NP's are on call 24 hours a day, seven days a week to respond to requests and care for our patients. No request is too great for our dedicated team
Patient and Family Education – Our dedicated NP's provide in-depth education on the plan of care to each patient and their loved ones. We ensure that every individual involved in the care of the patient understands and is comfortable with the plan in place
Culturally-appropriate approach to care – We employ English, Spanish, and Vietnamese-speaking medical staff to provide culturally-appropriate care to our diverse group of patients
CHAP Accreditation – Our hospices are accredited by the Community Health Accreditation Program (CHAP)
Our hospice team of physicians, nurses, social workers, home health aides, therapists, spiritual counselors and volunteers is highly trained in end-of-life care and works closely with each patient, their families and their primary care physicians. We believe in patient-centered and family-centered hospice care and respect the unique needs of each patient.
